There's a decal on it that lists a bike shop in Chicago, where the bike was bought. Doubt it's still around though.

I just called the number on the awning in the picture: disconnected. Maybe recently since no one else has the number yet. Ed

Sounds like it wasn't all that great of a place in the end... which is really too bad! It would have been cool to go there and say "hey look, my grandma bought this bike here back when the bike was new, and here's your sticker" (my mom remembers her getting the bike, and they did used to live in that area, so yea).

Very doubtful. What is the address ? Paint looks wonderful. Rare and shiny Medium blue. I just bought a lady's full size in the same colour. not sure if mine is a 76, can not read the date on hub. I would guess yours is a 69 with the type of handlebars. Swap is always possible in the past of course.

I doubt anything was swapped. The address isn't readable in the pic, even zooming in on my phone, but I took a stab at the name and may have guessed right. It appears to be Art's Cycle & Hobby, which may still be around (not sure... seems to be a really old page I found?): http://www.chicagobikeshops.info/shopDetails.php?id=12 I'll have to do a little more investigating I guess!
